- Care of Medically Fragile Individuals: For those requiring more than the usual help, we structure the plan to reflect complex needs, including therapies, nursing oversight, medical equipment, and continued monitoring.
- Flexible Care Hours (15-50 hours/week): Whether your loved one needs part-time support or near full-time oversight, your Plan of Care lays out the weekly hours clearly, with flexibility for changes.
- Caregiver Compensation Strategy: We explicitly define you as the paid care provider. The plan shows how your hours, services, and role translate into caregiver compensation through the program.
- Customised Schedule & Routines: We tailor when and how care happens, whether that is mornings, evenings, weekdays, or weekends. The plan is designed to adapt to your family life, home rhythms, and local community norms.

Our Plan of Care Development Process

Contact us by phone at (505) 578-7535 or via our website to schedule a no-obligation telephonic consultation. During this call we’ll gather your story, understand your caregiving role, and begin to identify how the hours of care, your loved one’s needs, and your goals might align.
We work closely with your family, the care recipient’s physician, and state care-coordination staff. Your insights as the primary caregiver is vital in this process. Together, we will conduct an initial review of medical history, functional mobility needs, home safety, and so on.
Our care coordinator will then visit your home (or via video if needed) and complete a detailed assessment. We evaluate the full range of needs including ADLs like bathing, dressing, feeding, toileting and mobility, as well as scheduling realities and caregiver capacity.
We craft a customized Plan of Care listing services, hours (15-50/week as needed), and how it fits under Turquoise Care. The document is then forwarded to your case manager along with budget forms and all documentation required for your caregiver pay authorization.
Once approval is secured, you begin in the caregiver role, guided by our team. We help you set up timesheet tracking and set up payment mechanisms, ensuring your caregiving hours convert into the compensation you deserve.
Care needs may change over time. Each quarter (or sooner if needed), our care coordinator reviews the plan with your family. We make adjustments to hours, services, and caregiver compensation as per evolving needs. We ensure that your loved one’s care stays current and you stay supported at all times.
